Disability waiver program for children and adults in Minnesota

The Individualized Home Support service is a program offered to children and adults with disabilities that provides support and training in the following community living services areas:

  • Community participation
  • Health, safety, and wellness
  • Household management
  • Adaptive skills

This service has three types namely:

Without training
This is ideal for adults or children needing support in at least one of the community living services areas mentioned above. The services offered to clients can include adult companionship and personal support.

With training
This is ideal for adults who need support and training in at least one of the community living services areas mentioned above. The services offered to clients can include independent skills training and supported living services in their respective homes.

With family training
This is ideal for children and adults who live with their families and need support and training in at least one of the community living services areas mentioned above. Family members receive training to enhance their skills and understanding of the level of care their loved ones need in their respective homes.
